HEARING AID SERVICE PLANS BREAKDOWN
YEARLY Subscription Services
Available in annual, bi-annual, and quarterly options. All subscriptions come with waived transportation fees. All subscriptions are set to auto-renewal status, and cancellation of subscription requires a 30-day notice. Refunds on subscription services are granted within the 45-day period from the initial HA fitting, with a 15% service fee. All services established after 45 days are set to an annual basis, and cannot be cancelled till one-year from the initial purchase.
Tier One: – The Habilitate
$1000/annual or $700/bi-annual or $400/quarterly 6 visits per year – in-person/ virtual
Included services:
o Initial comprehensive audiogram
o Speech-in-noise assessments
o Prescriptive based HA fitting
o Functional test measurements
o Subjective survey measurements
o Deep HA clean and checks
o Unlimited in-house/in-warranty repairs, during active subscription
o Cerumen management
o At-home canalith repositioning procedures (CRP)
o At-home HA accessory connection assist
o Annual audiologic assessment
o Annual HA hardware and firmware updates
o Real-ear measurements
o All HA cleaning supplies
o Group aural rehab classes
Tier Two: – The Synergy
$600/annual or $350/bi-annual or $210/quarterly 4 visits per year – in-person/ virtual
Included services:
o Initial comprehensive audiogram
o Speech-in-noise assessments
o Prescriptive based HA fitting
o Functional test measurements
o Deep HA clean and checks
o Unlimited in-house/in-warranty repairs, during active subscription
o Cerumen management
o Annual audiology assessment
o Annual HA hardware and firmware updates
o Real-ear measurements
o Group aural rehab classes
Tier Three: – The Advocate
$450/annually 3 visits per year – in-person/ virtual
Included services:
o Initial comprehensive audiogram
o Prescriptive based HA fitting
o Speech-in-noise assessments
o Functional test measurements
o Deep HA clean and checks
o Real-ear measurements
o Group aural rehab classes
o Annual audiologic assessment
o Annual HA hardware and firmware updates

o Tinnitus Diagnostic Evaluation, with waived evaluation fee
